The book was never very clear about Winnies relationship with John. She keeps a fictional character of him as her spouse, but we know that John is her step-cousin. What is this "relationship" that keeps creeping up through Winnie's intimacy and jealousy around John?

The story mentions Winnie having a baby and about the baby starving in the cold. It's hard to say if her husband is now dead or that she is dead to him, but she still cared for him enough to keep his picture (that did not look like John). Still, the story tells us that John was there when the baby died. Was Winnie having an affair with John? How did Winnie's baby die?

no, no. winnie didn't have a baby, she was adopting one with her husband. her step-cousin john went along with her b/c her husband couldn't make it. Her & john ended up having an affair on the trip to pick up the baby & there was a storm (winnie & john got stuck in the hotel) and by the time her and john got to the place where the baby was, it had died.
